Please read the consent forms for the processing of personal data which must be completed by MultiSport and/or MultiLife Program Users at your company. Every submitted Employee, Accompanying Person, person over 60 years of age, as well as child over 18 years of age and child up to 18 years of age (in the latter case the consent must be signed by the child’s parent or legal guardian) must sign the documents in order to be able to use the MultiSport and/or MultiLife Program.
